Various cleanups. Explicitly include <sys/atomic.h>, Use pmap_remove_pg()
for the kernel pmap and kill pmap_kremove_pg().  Finally guard the hash
lock code under "MULTIPROCESSOR" to explicit which part of the code
received some MP love.
